If your child is already a patient at De Kinderartsenpraktijk, you can request refill prescriptions by calling or emailing the location where your child receives care. Click here for contact information by location.
Authorizations for Special Diets
Some children receive special (bottle) formula, such as cow’s milk-free formula for a cow’s milk allergy, or high-energy formula for growth failure. If your child is receiving special formula through De Kinderartsenpraktijk, your pediatrician will determine how long this is necessary. The pediatrician can only prescribe a maximum amount of formula. If you run out before the agreed-upon period ends, you’ll need to purchase additional supplies yourself. Together with your pediatrician, a decision will be made on whether the authorization period needs to be extended. An appointment will be scheduled for this purpose. If you have any additional questions, you can request an extension by calling or emailing the location where your child receives care.
